For the Scholarship Exam Qualifiers, you may follow the steps below:

  1. Save/Download a copy of this form into your computer.
  2. Send an email to scholarship_office@uic.edu.ph with the following information:
    1. Subject (whichever applies):
      1. UIC SCHOLARSHIP EXAM - PASSER 65 - 72 (ALL CAPS)
      2. UIC SCHOLARSHIP EXAM - PASSER 54 - 64 (ALL CAPS)
    2. Body of the Email:
      1. Name
      2. School Last Attended (Grade 12)
      3. Exam Date
      4. Testing Center (school where you took the scholarship exam)
    3. Attach the following documents:
      1. This Form (screenshot)
      2. Scanned Copy / Picture of Report Card (with most recent grades)
    4. Submission / Application deadline is on June 20, 2025.

    5. You will receive a confirmation email with the corresponding scholarship grant not later than June 30, 2025.
