To avail of the scholarship grant, you may follow the steps below:

  1. Save/Download a copy of this form into your computer.
  2. Send an email to scholarship_office@uic.edu.ph with the following information
    1. Subject: UIC SCHOLARSHIP EXAM - PASSER 65 (ALL CAPS)
    2. Subject: UIC SCHOLARSHIP EXAM - PASSER 56 (ALL CAPS)
    3. Body of the Email:
      1. Name
      2. School Last Attended (Grade 12)
      3. Exam Date
      4. Testing Center (school where you took the scholarship exam)
    4. Attach the following documents:
      1. This Form
      2. Scanned Copy / Picture of Report Card (with most recent grades)
  3. Submission / Application deadline is on July 3, 2023.
  4. You will receive a confirmation email with the corresponding scholarship grant on July 17, 2023.
Cut-off Scores:

65 - 72 = 80% to 100% Tuition Fee Discount

54 - 64 = 40% to 50% Tuition Fee Discount

45 - 53 = 30% Special Tuition Fee Discount (if you will take any of the courses under CABE, CAH, CEA, CTE; HRIM and TM

30 - Admission or Score Cut-off (Entrance Exam)
